POLICIES for ACUPUNCTURE | HERBS | NUTRITION



Reservations
Call 312-335-9330 for an appointment. A credit card number is required to hold your reservation. We will work to accommodate your time of availability.

Forms of Payment
Source Healing accepts payment by cash, check, visa, or mastercard. Payment is required at the time service is provided.

Cancellation Policy/Missed appointments
As a courtesy to our practitioners, please call (do not email) to cancel at least 24 hours prior to the time of service. If your appointment is not cancelled within 24 hrs, you will incur the full charge of treatment. This policy also applies to missed or “no show” appointments.

Lateness
As a courtesy to all of our patients, we begin appointments on time. If you are running late, we will provide the best treatment for you in the remaining time allotted.

Insurance
Source Healing requires full payment at time of service. However, if your insurance accepts acupuncture, we will provide the necessary information for you to send to your healthcare provider so that you may obtain reimbursement directly. Please note that it is the responsibility of the patient to determine whether his/her insurance company includes acupuncture treatment in his/her medical coverage and to seek reimbursement from the insurance provider. Upon payment, Source Healing will provide you with a copy of the bill ("super bill") and an itemized receipt to submit to the insurance provider.

Flexible Spending Accounts (FSA)
If you have a corporate FSA that covers acupuncture we still require full payment at time of service. Like our insurance policy, we will provide an itemized receipt for reimbursement. Please check with your human resources representative for details.

What to bring
Except for payment and filled out forms, you do not need to bring anything on your treatment day. Most medical information will be addressed in our intake form. If you are coming in for a specific injury or pain management issue, you can bring any medical records pertaining to that injury if you have it, but it is not necessary.

What to wear
For acupuncture, no special clothing is required on the day of your treatment. For a mini acupuncture session or Asian Bodywork, wear comfortable loose-fitting clothes.

Before and after treatment
Before treatment, it is wise to have something light to eat. After treatment, it is normal to feel relaxed or light-headed immediately following a treatment. Most people go about their normal work day after a treatment.
